To make a referral to Veritas House’s Clinical and Consultation Services (CCS) team, please complete the following referral form. Please ensure that consent to send this referral is gained from the client and/or parent, guardian or carer prior to submitting. If we require further information, one of our staff will contact you within 3 working days.

Please note, submission of this form does not constitute an acceptance of the referral by CCS.

The full service fee of $210/hour (ex GST) will be charged for a scheduled service if it is cancelled with less than 24 hours notice or for non-attendance by the service recipient.
